Our church introduced us to this. We keep a copy for each of us on the refrigerator in our RV.

We got holders at our local fire station, but I will have a supply at the National Rally in Rapid City this September if you'd like one.

These come with cards for you to fill out. It's hard to fit everything in and my handwriting is getting worse as I age, so I created a fillable PDF form. The form is attached below. It's easy to fill out and keep up-to-date.
