Pretty neat. I would guess that it came from the factory this way.
Note that the access door in the back.
It is for sale and would be ideal for someone needing such a vehicle.
Only 25,600 miles.
And looks to have a Handicap Shower.

The factory can do all kinds of customizations for accessibility, based on the production models. We worked with John Eastman at the factory to design our unit, which made it possible for us to travel places we haven't been able to visit in years.
